Covid-19 Liability Release Form

This form is required prior to every scheduled appointment.

Here at Brazilia Skin Care, we are now taking safety precautions to protect our wonderful clients and employees by implementing additional sanitation and disinfection practices that meet and exceed the CDC guidelines. Please read and complete this information before any scheduled appointments, and electronically sign the form below.

Possible symptoms of COVID-19 include:

  • Cough
  • Shortness of breath or difficulty breathing
  • Fever
  • Chills
  • Muscle pain
  • Sore throat
  • New loss of taste or smell

By signing below, I agree to each statement above and release Brazilia Skin Care from any and all liability for the unintentional exposure or harm due to COVID-19. All Brazilia Skin Care staff agrees to abide by these standards and affirm the same.

Some of the precautions Brazilia Skin Care is following to prevent the spread of COVID-19 include:

  • Prepayment for services or contactless checkout
  • Additional time included between appointments to implement strict sanitation guidelines
  • Guests accompanying those scheduled for services discouraged unless the client is a minor
  • Your esthetician will wear a mask and thoroughly clean hands before and after each service and wear nitrile gloves for waxing
  • All surfaces will be wiped thoroughly with hospital grade disinfectant before and after each client according to the manufacturer’s directions
  • We will not be operating at full capacity in order to abide by California State regulation rules. Appointments are required and will be limited, as no walk-ins are accepted at this time. We encourage online bookings.
